SMS ENABLED CONTACT FORM FROM EASYKISS123.COM

CHANGES TO BE MADE IN SMSFORMPROCESS.PHP INSIDE THE INCLUDES FOLDER
1. on line 15 add your own email
2. on line 17 change it to the subject you want sent to your email.
3. On line 98 change it to your own email address
4. On line 110 change it to your phone number as an email address with your cell phone provider gateway.
5. Line 130 Change it to your own email address 


CHANGES TO BE MADE TO THE PAGE YOU WANT THE CONTACT FORM ON
1. Insert the line of code below at the very top of the page you want the contact form on.
It must be at the very beginning of that page with no spaces at all.

2. Insert the line of code below into the head section of the page you want the contact form on. It is the
Style sheet that gives the form it's look

3. Make sure your page has is named as a PHP file for example contact.php instead of contact.html




4. Paste the code below into area on your page you want the form to show up on. Then upload the folder called smscontactform.
